Community Transitions is a Day Support facility in which adults with disabilities are given support to develop skills that assist with their daily life. All individuals enrolled in Community Transitions are provided with individualized programming that will increase vocational, educational, social and personal care skills.
Community Transitions works with several volunteer agencies to provide on the job training and social awareness. As a part of Community Transitions all individuals served may choose to participate in our education workshop where they will be given support to expand their academic levels and communication skills.
Community Transitions Day Support works in conjunction with our clients, families, and community to enrich and improve the quality of life for each individual we serve.
Educational Workshop
Overview:
Our main objective is to create a structured learning environment conducive to improving community, survival/functional living and vocational skills. Community survival and functionality will be the emphasis of all activities. Each consumer participating in this program will be assessed for skill level and assigned appropriate goals to work towards. All consumers will be given the opportunity to be actively involved in this process. After the assessment is completed, each consumer will be placed in a need based program. Each consumer will fall under one of three categories: Vocational, Pre-Vocational, or Survival/ Functional Living Skills.

We encourage all consumers and their families to express any activities, goals or personal desires they feel could enhance each workshop experience. All classes will be focused on community and personal goals in order to enhance overall development. All activities are designed to encourage independence and personal success. All pre-vocational and functional living skills will be integrated with vocational level training as needed.
